
Shelter Plus Care Application
Homeless adults with disabilities are eligible for Shelter Plus Care, which provides permanent housing and supportive services.
Homeless - To qualify for the program, a homeless individual is one who lacks a fixed, regular and adequate nighttime residence, and has a primary nighttime residence that is:
- A supervised publicly or privately operated shelter designed to provide temporary living accommodations (including welfare hotels, congregate shelters, and transitional housing for the mentally ill), or
- An institution that provides a temporary residence for persons intended to be institutionalized, or
- A public or private place not designed for, or ordinarily used as, a regular sleeping accommodation for human beings (street, park, hallway, or underpass).
Disabled - Shelter Plus Care primarily targets people who are diagnosed with one or more of the following: Chronic alcohol and/or substance abuse, severe mental illness, Aids and related diseases.
